Use the "Save" Button to download the pdf file to your computer

When the download is complete it will ask you where to save the file.

Select a location on your computer and save as a pdf file.

Exit the NUMC website when you are finished.

When printing, set your printer for 2 sided or duplex printing in landscape mode.

We recommend using paper better than standard to reduce shadows.

Fold printed form in thirds to complete with "You are invited" on the front.
